What Is An Enhanced Primary Care Plan (EPC)?
An Enhanced Primary Care (EPC) Plan – now commonly referred to as a GP Management Plan or Chronic Disease Management (CDM) Plan – is a Medicare-funded initiative in Australia that allows people with chronic medical conditions to access allied health services (like Chiropractic care and others etc.) at a subsidised rate.
- Developed by your GP to help manage a chronic or long-term medical condition (e.g., arthritis, diabetes, asthma, heart disease, chronic pain, etc.).
- Provides a structured approach to your health care.
- Involves referrals to allied health professionals who help support your treatment and improve your quality of life.

Who Is Eligible For An Enhanced Primary Care Plan (EPC)?
You may be eligible if you:
- Have a chronic condition that has lasted (or is likely to last) 6 months or more.
- Would benefit from a multi-disciplinary care team, including services outside your GP (e.g., physio, chiro, dietitian).
Your GP will decide if you are eligible and whether an EPC/CDM Plan is right for your situation.
What Do You Get?
- Access to up to 5 Medicare-subsidised allied health visits per calendar year.
- These visits can be split between different providers (e.g., two physio, three chiro), depending on your care needs.

Here’s how you can get started in 7 steps:
- Talk to Your GP
Book an appointment with your GP and chat about your condition. Whether it’s back pain, arthritis, or another ongoing issue, your GP can assess if an EPC plan is right for you.
- Work on a Care Plan Together
If you're eligible, your GP will put together a Chronic Disease Management (CDM) plan. This outlines the best way to manage your health and includes referrals to allied health professionals, like Chiropractors.
- Get Your Referral
Your GP will provide a referral form for Medicare-subsidised allied health services. Be sure to hang onto this—it’s your key to accessing discounted care!
- Choose Your Chiropractor
Find a chiropractor who accepts EPC referrals (like us!). If you’re unsure, give the clinic a quick call to confirm any out-of-pocket costs and we'll discuss how the plan works.
- Make Your Appointment
Once you have your referral, call your chosen chiropractor to book an appointment. With the EPC program, you may qualify for up to five Medicare-subsidised visits per year across different allied health services.
- Start Your Sessions
Bring your referral to your first session and start your journey toward feeling better. Your Chiropractor will also keep your GP in the loop about your progress.
- Follow Up with Your GP
After you’ve used your referred sessions, it’s a good idea to check in with your GP. They can review your progress and decide if more care is needed.
